Long Life Care Management is a faith-based organization. We provide compassionate, timely, and individualized consultations with valuable clinical collaboration. Caregiver risk and burden assessment is a high priority for us. Our chronic, transitional, geriatric care management services enhance quality of life goals and client satisfaction, backed by years of experience in both outpatient and inpatient care. We are Aging Life Care™ Managers, qualified and licensed professionals.

Our clients chronically and seriously ill patients. They are considered vulnerable and at risk for hospitalization or acute changes in their condition. Chronic care involves two or more conditions that are expected to last more than three months. Seriously ill patients may have multiple comorbidities, with more than 50% experiencing depression, anxiety, and chronic pain. More than 80% of these patients defer their care needs or care planning due to psychosocial and financial barriers. Your care matters!
Our care is accessible to clients, caregivers, families, and collaborative clinicians. Consultations are conducted by an Aging Life Care™ Manager, a Board Certified Practitioner, an experienced Certified Hospice & Palliative Nurse, and a Nurse Educator with Veterans Affairs Care Manager experience. We deploy a collaborative team approach that includes licensed clinical social workers and clinicians in long-term care, hospitals, hospice, and home-based care settings. Our network is connected to elder professionals, healthcare agencies, attorneys, and community services both within and outside the service area.
